Alateen Forms & Guidelines
The following forms and documents are used in the Oregon World Service Area of Alanon Family Groups to protect the safety of Alateen members. If you have any questions please contact our Oregon Alateen Coordinator at alateen@oregonal-anon.org.

Eligibility Requirements to Apply
Alanon members must meet the following eligibility requirements to apply to be an Alateen Sponsor.
Be an Al-Anon member regularly attending Al-Anon meetings.
Be at least 21 years of age.
Have at least two years in Al-Anon in addition to any time spent in Alateen.
Not have been convicted of a felony.
Not have been charged with or convicted of endangering a minor, child abuse or any other inappropriate sexual behavior.
Not have demonstrated emotional problems which could result in harm to Alateen members.
Disclose to the AAPP any involvement as the subject of an investigation by a child-welfare or similar government agency.
Pass a criminal background check as a new applicant and once every 3 years thereafter
Application Overview
Alanon members who wish to serve Alateen in any capacity are required to go through a certification process which includes interviews, references, fingerprinting, a background check and a training process. Once certified, the member becomes an AMIAS (Alanon Member Involved in Alateen Service) and may participate in the full capacity of an AMIAS.

Send the completed documents from steps 2 and 3 to your District Rep. If there is no District Rep, send to the AAPP.
Applicants who are planning to sponsor an Alateen Group are to be certified in the same district as the Alateen Group. All other applicants are to be certified in the district of their Alanon home group.

3 Verify the documents have been correctly filled out and signed
4 Interview candidate and call references.
Fill out the DR Review of AMIAS Candidate (1 page) form.
5 Send all completed, signed forms to the AAPP

$64.25 check for the background check payable to “Oregon Area” to the AAPP’s personal mailing address
6 Let the AMIAS candidate know
They will be hearing from the AAPP with instructions for fingerprinting
The candidate pays fingerprinting fees ($12.50) and must send a copy of the receipt to you (the DR) to reimburse.

The AMIAS must ensure that the following forms are filled out correctly according to the instructions on the documents, and remain in possession of the form(s) while the Alateen is in their charge (if parent or guardian not present).
Alateen Permission Form - Must be filled out when providing transportation for an Alateen and/or when Alateen attends an event without a parent/guardian.
Alateen Medical Form - Must be filled out for an Alateen attending an event without a parent/guardian.
Note: For Alateens attending the Oregon Alateen Conference (OAC), the registration packet includes all necessary forms to meet the above expectations.
